The Completing the Reasonable Accommodation Request Form
Here's how to complete the Reasonable Accommodation Request Form. All Information must be completed, otherwise your application will be Deferred.​​

Supporting Documents
Please be ready to provide any supporting documents. If you are making a request that involves an animal, please complete the Animal Information.

Therapeutic Relationship Verification
The next step in the approval process will verifying your information with your Medical Provider.

Review Process
Once we have received all your information and supporting documentation, it will be reviewed by our Legal Team. If there are any questions, they will be contacting you directly.

Approval Process
You will receive an Approval or Denial notification within 10-14 business days.

If Approved, the final step in the process will be signing the Reasonable Accommodation Request Addendum and if it relates to a Service Animal; you will be issued a Property Collar.

Note: If your request is for an Emotional Support Animal, until it is approved- it will be processed as a Pet under the Lease and Rules and Regulations.
